The membership application is provided in two different formats: an Adobe PDF Form, and a Microsoft Word Document. Please follow the instructions corrisponding to the format you choose.
Instructions - PDF:
You must have Adobe Acrobat Reader (version 8.1.1 or greater) installed on your system to fill out the PDF version of the membership application. You may download Adobe Acrobat Reader for free from the following link.

Open the PDF application in your browser from the link below.

Print the filled in form by clicking the print button on the tool bar at the top of Acrobat Reader.

Fill in the form with your information (Required fields are outlined in red).

Choose the Adjusted Dues corrisponding to the month you will begin your membership (choose the month you will attend you initial membership meeting.)

Check the form to make sure you have filled out all the required fields.

Either bring the printed application with a check for the total amount due to the next membership meeting, or mail them to the P.O. Box below (the P.O. Box is also listed on the form.)
Note: You must still attend the membership meeting, even if you mail in your application and dues.

You must have Microsoft Word installed on your system to open and print the MS Word version of the membership application. (If you do not have MS Office or MS Word, please follow the instructions above for the Adobe PDF version.

Download the MS Word application to your desktop from the link below (right click and choose Save Link As... or Save Target As....

Open up the application in MS Word and print it out.

Fill in the form with your information (Required fields are marked mandatory).

Fill in the Adjusted Dues field with amount corrisponding to the month you will begin your membership (choose the month you will attend you initial membership meeting.) Add the Initiation Fee and the amount in the Adjusted Dues and write the sum in the Total Submitted field directly below.

Check the form to make sure you have filled out all the required fields.

Either bring the filled in application with a check for the total amount due to the next membership meeting, or mail them to the P.O. Box below (the P.O. Box is also listed on the form.)
